Summary of QRIS Monitoring Visit: The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and scoring of the QRIS. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. Fire drills not conducted monthly. Not all Items labeled "keep out of the reach of children" are locked when not in use with the exception of hand soap. Child records and application was not complete for all children enrolled Staff records were not complete and accurate Menu should be CACFP complaint Lesson Plan does not meet TNELDs for activities Director did not follow required process for reporting serious incidents to DHS and took actions that interfered with a DHS investigation. Fire drills not conducted monthly.

Summary of Complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and Complaint visit for PCS1003814. Technical Assistance Provided: Discussed children should not wear paci clips or necklaces. Discussed staff physicals should be onfile within 10 days after start date. Discussed trainings and reminded them to make sure all staff has licensing and regulation training completed in prosolutions. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. 3 educators missing phyisical forms on their files on site. observed 1 child in younger infant room wearing paci clip. observed 1 child in older infant room wearing necklace.
