Summary of Complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations to investigate a complaint. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. During an unannounced visit on May 13, 2026, the Licensing Consultant observed one educator and seven children in the one-year-old classroom. The classroom was out of compliance with the required adult-to-child ratio. During an unannounced visit for a complaint investigation on 5/13/2026, the licensing consultant confirmed that an incident was investigated, but no incident report was completed.

Summary of QRIS Monitoring Visit and Complaint Investigation The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and scoring of the QRIS and a complaint was investigated. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. This indicator was not met because the educator left a child in the classroom for a few seconds. During an unannounced visit on April 1, 2026, the licensing consultant confirmed that a child was left unattended in a classroom for approximately one minute. During an unannounced visit on April 1, 2026, the licensing consultant confirmed that the educator did not use a name-to-face roll call when transitioning the children from the classroom.
