See Below for Detailed Information Observation: The director self-reported that an educator taped a three year old's wrists together as discipline for the child not listening to her. The incident occurred on 9-25-19 and was cited on 10-4-19. 1240-04-01-.15 PROGRAM, LANGUAGE, AND LIERACY DEVELOPMENT Corrective Action: Program Evaluator provided technical assistance. A Plan of Corrective Action was completed. The agency first suspended and then terminated the employee for this incident. The director retrained all remaining staff on child care licensing discipline rules and the agency's positive guidance policy. Program Evaluator will do training with all staff on appropriate discipline techniques.

The staff saw the child standing at the entrance of the agency and returned the child to his classroom. 1240-04-03-.06 SUPERVISION Corrective Action: The center followed correct protocol after the incident. They contacted the child's parents, the Department of Human Services, the Department of Children's Services, their corporate headquarters, and the National Head Start Association to inform them of the incident. Staff responsible for the incident received disciplinary action. All staff received supervision training again to ensure this never happens again. The child was moved to the building where his brother is located.

1240-04-01-.11 SUPERVISION. (1) Supervision. When children are not within the direct sight and sound of an adult, the term "supervision" means: (a) For children six (6) weeks of age through nine (9) years of age, the adult must be able to hear the child at all times, must be able to see the child with a quick glance, and must be able to physically respond immediately (2) Supervision Procedures.
