Summary of Complaint Visit
The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint investigation.

During the visit on May 23rd, 2024, and May 24th, 2024, Child Care Licensing Consultant Lexi Casteel was investigating a complaint and determined an accident/incident form was not filled out for a child regarding an incident where a teacher snatched a toy out of a child's mouth. Lexi Casteel requested to view the report and it was determined one was never filled out and filed in the child's file.
During the visit on May 23rd, 2024, and May 24th, 2024, Child Care Licensing Consultant Lexi Casteel interviewed staff regarding a complaint that came in regarding supervision issues on the playground and in classrooms. Lexi Casteel received written statements from two staff members regarding a playground accident that was due to lack of supervision and teachers being on their cell phones.
During the visit on May 23rd, 2024, and May 24th, 2024, Child Care Licensing Consultant Lexi Casteel interviewed staff regarding a complaint that came in regarding supervision issues on the playground. Lexi Casteel received written statements from two staff members regarding a playground accident that was due to lack of supervision and teachers being on their cell phones and not following the playground supervision plan in place by the provider to actively move around and monitor all children while playing.
During the visit on May 23rd, 2024, and May 24th, 2024, Child Care Licensing Consultant Lexi Casteel was investigating a complaint and reviewed cameras regarding an incident that took place on May 8th,2024 at 4:15 PM. Lexi Casteel viewed a staff member walking aggressively to a child and storming off. After interviewing staff in the room with the educator it was determined that the staff member aggressively yanked a toy out of a child's mouth after another teacher told him that we do not put things in our mouths.
On May 23rd, 2024, and May 24th, 2024, Child Care Licensing Consultant Lexi Casteel was investigating a complaint and determined an incident was not reported to DHS Licensing. It was determined the director was aware of the incident and did not report it.

Observation: On 5/22/18 during the assessment, a child was served a hot dog from home that was not cut up. The child quickly ate the hotdog before it could be addressed.

Plan of Correction: Agency is aware of foods that are a choking risk and need to be cut by licensing rules. Parents have also been notified of choking risks and asked to cut foods at home and that if foods require additional cutting they may be served a later in the meal when the educator have time to cut them properly.
