Summary of Complaint Investigation The visit conducted today was to investigate PCS1001999 Technical Assistance Provided: 1. Stop transporting immediately. LC provided information on how to become authorized to transport as primary educator requested this information. Violations were received and include: 1. Transportation violation. Correct Action Plan (CAP) will be completed. LC determined agency was conducting unauthorized transportation.

See Below for Detailed Information OBSERVATION: On 8-18-2017 it was observed that a caregiver was attempting to remove two children from the agency without the parents' permission. The caregiver stated the owner told her to remove them so they would be within ratio requirements. 1240-04-01-.02 Ownership and Administration-Enrollment of Children and Parent Involvement OBSERVATION: On 8-18-2017, a partially consumed bottle was left in a bouncy seat and after being outside and entering the day care the child picked the bottle back up and started drinking it again. The contents should have been discarded after consumption as required by rule. 1240-04-01-.08 FOOD OBSERVATION: On 8-18-2017 it was observed that children's attendance records, excluding the ones for 8-18-17 and 8-17-17, were not available to Program Evaluators as required by rule. 1240-04-01-.02 OWNERSHIP AND ADMINISTRATION OBSERVATION: On 8-18-2017 the ratio was 2:15, all preschoolers, with 7 children under 2 years. There should have been 3 caregivers present and no more than 12 preschoolers. 1240-04-01-.03 STAFF OBSERVATION: It was observed on 8-18-2017 that with two caregivers present there were fifteen (15) pre-school children present with more than four of them being under (2) two years old. The agency was over by 3 preschool children and there should have been 3 caregivers present.. 1240-04-01-.03 STAFF
