See Below for Detailed Information Observation: The toddler room was out of ratio as there was one educator with nine children. For nine children in this age group there needs to be two educators. Violation: 1240-04-01-.22 SPECIFIC RQUIREMENTS FOR CHILD CARE CENTERS. Plan of Corrective Action: The director will ensure all rooms at all times are within the ratio requirements. Observation: A two year old child was left unattended restrained in a high chair. Violation: 1240-04-01-.15 PROGRAM, LANGUAGE, AND LITERACY DEVELOPMENT Plan of Corrective Action: Educators will be trained on the appropriate ways to use restraining devices. Observation: A television was on in the toddler classroom for a large part of the day. Violation: 1240-04-01-.15 PROGRAM, LANGUAGE, AND LITERACY DEVELOPMENT Plan of Corrective Action: The director will go over the rule (with educators) about no television in rooms having children under two years old and enforce the rule. Observation: The educator in the toddler room was very focused on her activity while sitting on the floor in the room. She was not able to see all the children when continuing to look down toward the shelf she was arranging and did not visually scan the room or otherwise attempt to observe the activities of the children in her care. Violation: 1240-04-01-.11 SUPERVISION Plan of Corrective Action: The director will continue to work with the educator to increase her supervision skills.

Compliance: In order to maintain compliance the center should have immediately repaired or replaced the equipment. A one year old child was asleep on a mat that was less than the required thickness of 2 inches. 1240-04-01-.14. EQUIPMENT FOR CHILDREN. Compliance: In order to maintain compliance the center should have ensured to have and use nap equipment that is at least 2" thick.
