Summary of Complaint Investigation Visit. The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and Complaint Investigation Visit. A 4 year old child was left in the classroom bathroom when the class left the room. The educator in the 4 year old room did not do the name to face roll call prior to leaving the classroom which resulted in a child being left in the classroom bathroom.

LC reviewed video footage of a child being on the receiving end of inappropriate discipline.
LC walked in to take ratio and the sleep timer was going off. The educator reset the timer, then proceeded to sit on the floor to play with the infants that were playing on the floor. The educator never checked the 2 sleeping infants that were asleep in their cribs.
There was no educator actively supervising the children playing on the climbing structure which resulted in a child being injured.
