(1) Reports of Incidents, Illnesses, Accidents, Injuries, and Fatalities. (d) Incidents, accidents, injuries, and signs of serious illness to children shall be documented immediately with the following information: 1. Child’s name and date of birth; 2. Name of the person writing the report; 3. Date and time; 4. Date and time of completion of incident report; 5. Description of incident and circumstances; and 6. Action(s) taken by the child care agency. Observation: PE observed the child’s file did not have an incident report regarding the occurrence on 3/8/19. Compliance Statement: To ensure compliance, the agency will ensure that no children will be allowed to be unsupervised while on the playground. The agency will ensure that the whereabouts of the child is known at all times. Observation: A child was left unattended on the playground for 5 minutes. 1240-04-01-.11 SUPERVISION 1. Supervision Procedures (c) If any child is left unattended at any time, the child care agency is subject to summary suspension, denial, or revocation of the license. This includes but is not limited to a child: 3. Being left on the playground. Compliance Statement: To ensure compliance, the agency will ensure that no children will be allowed to be unsupervised while on the playground.
