1240-04-01-.14 Equipment for Children Observation: A door in one of the classrooms leading outside needs to be repaired. The door can be pulled open without even turning the knob making it easy for children to get out of the building. Shelves were also not secured and could easily be tipped over or rocked. Corrective Action: The educator in charge stated she would talk to the director immediately about the issues so they could be taken care of. On 11-21-18, an email was received at 6:38 AM stating the door had been fixed. Photos of the door fixed were received on 11-27-18. Photos of the shelves being anchored with a L shaped bracket were received on 11-29-18.

Pea gravel is used as resilient surfacing, but it is not 6 inches in depth and does not cover all fall zones. There was concrete showing from under the pea gravel as well as wood was sticking out of the pea gravel. Plastic retainers are used but not placed properly. Corrective Action: Agency anchored the metal school bus. Agency placed pea gravel under the climbers and slide. Agency also properly place plastic retainers.

During unannounced complaint visit wheras it was alleged that a parent/guardian came in around 01:00 and no adult/caregiver was supervising the children, when ccpe Linda Lloyd arrived at this agency at approx 01:04 I found the same situation as reported in that there was no caregiver in the room where the 3 year olds were sleeping. To be in compliance a caregiver must be in the room supervising the children and also able to physically respond if needed. CCPE found no caregiver in the room. It was noted that the caregiver for this room did "peek around the corner" at this worker, but never asked if she could help me or ask me what ccpe was doing in this room. Agency was cited for lack of supervision on check sheet, 1240-04-03.06 1 b. Supervision procedures. Linda Lloyd Put on notice was mailed on 09/08/2009. On 10/05/2009 put on notice not responded back on. Phone contact made this date to Ms. Grant advising her unless this is received by email today, another notice will be sent for failure to follow through on an cited complaint.

There were eight (8) children in this room, and a careful search of the room did not reveal any staff either sitting with the children or in the bathroom located in the room. During an approx. 5 minutes wait ccpe Linda Lloyd did not an adult in the next room come visible in the doorway of the 2 year old room, but no one ever inquired what was needed, or why CCPE was in the building. After 5 minutes ccpe went over to the next building and knocked on the door and requested Ms. Grant come to the main building. She told ccpe as soon as her 2nd caregiver came in from lunch she would be over. When ccpe returned to the main building the caregiver who had been visible for a short time in the doorway was eating her lunch sitting at a table in the room. When ccpe explained "supervision" rules she appeared unconcerned that she had been found not in the room supervising the children.

During unannounced complaint visit wheras it was alleged that a parent/guardian came in around 01:00 and no adult/caregiver was supervising the children, when ccpe Linda Lloyd arrived at this agency at approx 01:04 I found the same situation as reported in that there was no caregiver in the room where the 3 year olds were sleeping. To be in compliance a caregiver must be in the room supervising the children and also able to physically respond if needed. CCPE found no caregiver in the room. It was noted that the caregiver for this room did "peek around the corner" at this worker, but never asked if she could help me or ask me what ccpe was doing in this room. Agency was cited for lack of supervision on check sheet, 1240-04-03.06 1 b. Supervision procedures. Linda Lloyd Put on notice was mailed on 09/08/2009. On 10/05/2009 put on notice not responded back on. Phone contact made this date to Ms. Grant advising her unless this is received by email today, another notice will be sent for failure to follow through on an cited complaint.
