See Below for Detailed Information Observation: During a complaint investigation it was confirmed that an onion was fed to a child but the child was told it was an apple. This was done as a joke to see the child’s reaction. Staff 1240-4-01.06 Corrective Action. This staff is not longer with the agency. Staff were retrained on appropriate behaviors. Observation: During an unannounced complaint investigation visit, Licensing consultant (LC) Brooke Ownby observed an educator in the one-year old room who was not directly supervising the children during mealtime. One child was playing, one child was walking around the room while drinking from a cup, and the remaining four children were eating and seated at the table. The educator was completing paperwork while standing away from the table. Her back was partially turned, so she was not able to supervise the children during mealtime. LC Ownby immediately got the Director to go into the classroom and correct the supervision issue and provided technical assistance on proper mealtime supervision. 1240.04.01.11 Supervision Corrective Action: During meal time, an educator will directly supervise the children while they are eating.

See Below for Detailed Information Observation: As part of the complaint investigation it was found that on May 24 two educators had an altercation on the playground in which the director stepped in between the two educators while there was yelling and name calling. This was inappropriate action and unsuitable behavior in front of the children. It resulted in children talking to their parents about the behavior and being afraid to come back to child care the next day. 1240-04-01-.06 STAFF Corrective Action: Staff have left the agency. Agency will work on policy for discipline of staff who act inappropriately at work as well as ways of handling staff issues. Observation: As part of the complaint investigation it was found that on May 24, 2021 that educator of the 3 year old classroom left the playground and came inside the building to the office, leaving the 10 children from her classroom on the playground without a specific caregiver. The 4 yr. old educator was on the playground with the10 children from her classroom. She was not responsible for the 3-year-old class and would have been out of ratio if supervising both classes. 1240-04-01-.11 SUPERVISION. Corrective Action: Agency will receive new training on Supervision

See Below for Detailed Information Observation: Videos of several children had been taken on the educators' personal phones. The videos were sent to the parents of the children. One video, which was received by DCS, was video of a child in the restroom. The child was not wearing clothes when the video was taken. STAFF 1240-04-01.06 and PROGRAM 1240-04-01.15 Corrective Action: Agency reviewed with staff policy on phone use.

See Below for Detailed Information Observation: On 1/27/20 as verified on video educators were not supervising children resulting in children acting inappropriately and pulling down pants. 1240-04-01-.11 and 1240-04-04-.02 Supervision Corrective Action: All children will be supervised at all times. Additional training. Observation: Children were inappropriately discipline when time out was used too long. Children were held/restrained causing bruises. 1240-04-01-.15 PROGRAM, LANGUAGE, and LITERACY DEVELOPMENT Corrective Action: The staff shall use appropriate behavior management techniques. Observation: Class grouping were not met. Two children in the young 2’s class was under the age of 2. This class did not meet the 12 – 30 month group when there were 7 children present. 1240-04-01-.22 CENTER SPECIFIC REQUIRMENTS Corrective action: Agency will transition one child back today and then have two educators to meet ratios until all children turn 2 years old. Observation: Agency did not have a playground supervision plan. Two classes did not have roll call sheets and therefore did not do roll call today with transitioning to and from the playground. 1240-04-01-.11 SUPERVISION Corrective action: The agency will have a playground supervision plan. The classrooms will call roll as they transition to the playground. Observation: Educators and children did not wash hands prior to mealtime 1240-04-0113 FOOD and FOOD SERVICE Corrective action: The educators and children will wash their hands before meals.
