During a complaint investigation on 11/25/25 FS Pass and LC Spells reviewed video and educator statements of an incident that occurred on 11/21/25 where a 20 month old child and 21 month old child were unsupervised. The children were playing in a hallway, and went out of a door leading to the fenced in playground. The children were unsupervised on the playground for approximately 5 minutes. The classroom educator Ella Jones, combined her class with Charles Keith, Anita Morgan, and Alisha Morgan. Ella Jones did not provide the educators with a roster. When Ella Jones noticed two of her children were missing when she returned to take her children back to their classroom. Both children were found on the playground. On 11/25/2025, during an investigation of a supervision lapse, FS Pass and LC Spells reviewed video footage and documented incident descriptions from educators. While viewing video footage, we observed educator Ella Jones go to the playground to retrieve the children. She was walked quickly and took the children by the hand. The one and a half year old child fell when she grabbed his hand. She picked the child up off of the ground by his arm, lifting his feet off of the ground. During a complaint investigation on 11/25/25 FS Pass and LC Spells reviewed video and educator statements of an incident from 11/21/25 where two 19 month old child toddlers were unsupervised. The children were playing in a hallway, and entered a classroom with an open door. The educators did not notice that the toddlers had entered the classroom. They were in the classroom unsupervised for approximately 3 minutes. The educators for the children were Charles Keith, Alisha Morgan and Anita Morgan. During an unannounced complaint investigation on 11/25/25, FS Pass and LC Spells reviewed video and educator statements of an incident from 11/21/25 where two 19 month old child toddlers were unsupervised. The children were playing in a hallway and entered a classroom with an open door. The educators did not notice that the toddlers had entered the classroom. They were in the classroom unsupervised for approximately 3 minutes. Management didn't ensure the children were playing in a safe place and staff didn't follow safety protocols leading to 4 children being unsupervised. Management are responsible for insuring that staff are capable and trained and are responsible for an efficient operation of the childcare agency.

There were 3 areas in the kitchen (2) ceiling holes inside the food pantry. One of the areas was under the sink which has been repaired with foam seal. Additionally, the 1st floor has some visible leaks that could possibly come from the condensation of the AC Unit. The lower part of the facility also has some visible water leaks in the hallway from the ceiling. Plan of Corrective Action: The agency contacted Mayfield Pest Control. The company visited the grounds to complete a walkthrough to see what repairs are needed. The pantry was cleared out and mopped. A Dehumidifier system was installed and the maintenance team is working to prevent leaks. Ceiling tiles are being replaced. Recording time child was released Checked logs for the entire week. Found that one driver had not been completing the log properly at loading and unloading time. The driver only indicated a loading time for the first child that boarded and then drew a line down for the remaining children that loaded and the same was down for unloading a time indicated for the first child to unload and only a line drawn pointing down for the remaining children to unload. Tracy Bryant Contacted STS while pe was onsite to notify management of contracted transportation of issue with logs to correct immedidately.

Upon observation of an infant room a infants crib mattress was propped up with a wedge beneath. When asked the teaching staff and director if a doctor¿s not was on file for the wedge, the agency replied that they needed to check they know the child has reflux, upon review of the file no doctors note was found. The wedge was immediately removed and the parent contacted the parent said that she wouldn¿t get a note and that she understood that the wedge needed to be removed.

On 6/8/2025 at 5:15p.m., Linda Height, Educator, was providing supervision for two children (female 6years old and male 7 years old). Height left the children unsupervised in the classroom for 2 minutes, according to the video footage. On 6/8/2025, at 5:15p, there were two school age children (6yrs and 7yrs) in extended care services. The educator left the children unattended in the classroom. The lapse in supervision was for approximately 2 minutes.
