During unannounced visit, it was found that the agency was not maintaining infant daily logs appropriately. During unannounced visit on 12/16/25, it was found that the agency was using food as a reward for toileting toddlers. During unannounced visit on 12/16/25, it was found that the agency did not properly follow handwashing procedures during and after diapering/toileting children.
