When PE arrived at the program, program had school age children divided into two separate areas with only one teacher supervising. Teacher was unable to see each group. One group had 6 students in a room while another group had 5 students. Also, during the visit, PE observed a teacher leaving the two year old room to take child to the restroom. No other adult was in the classroom and teacher could not see the remaining students. 0520-12-01-.06 PROGRAM OPERATION. (1) Each program shall maintain a system that enables all children in the program’s care to receive a level of supervision that is appropriate to their age and their development, physical and mental status so as to ensure their health and safety and that allows program personnel to know the whereabouts of each child. (b) Children six (6) weeks of age through two (2) years of age: the adult must be able to hear the child at all times, must be able to see the child at a quick glance, and must be able to physically respond immediately.
