See Below for Detailed Information Observation: On 5.7.22, a parent reported to agency staff that their child made allegations that a staff member used improper discipline techniques. This allegation was not reported to the DCS hotline by the agency as mandated by law and child care licensure rules. 1240-04-01-.10 DUTY TO REPORT CHILD ABUSE AND NEGLECT. Corrective Action: The rules on Duty To Report Child Abuse and Neglect were reviewed with the agency. Agency staff are now aware they must report any and all allegations. Staff will receive updated training. During the announced visit on May 10 2022, Licensing Consultant Tracee Garner observed the toddler classroom with a ratio of 1:6 children with an age grouping of Eighteen (18) months to thirty five (35) months. 1240-04-01-.22 SPECIFIC REQUIREMENTS FOR CHILD CARE CENTERS. Corrective Action: The rule was reviewed with the agency for proper adult to child ratio and groupings. Agency noted that they will speak to the parent to have the child moved to the appropriate classroom. A transition plan will be made immediately.
