This was failure to report. 1240-04-01-.10 DUTY TO REPORT CHILD ABUSE AND NEGLECT. Corrective Action: TA was given to the director and all staff interviewed to remind them they are mandated reporters and must report any suspected abuse or neglect.

Observation: There was one infant observed on his stomach with a blanket bunched up under his face and chin. He was awake but had been asleep for 7-8 minutes according to the lead educator. Observations: PE observed the infant teacher changing a diaper. After the diaper change the baby was placed in the floor without washing the baby’s hands. PE asked her to wash the baby’s hands and she did this immediately. 1240-04-03-.10 Health & Safety Plan of Corrective Action: The infant teacher corrected this by immediately washing the baby's hands after the diaper change. Assistant director was made aware of this violation and said staff will be re-trained.

Observations: On March 22, 2018 PE observed infant teacher alone in the infant classroom with five (5) infants six (6) months to thirteen (13) months of age. It is not known how long the class was with one caregiver but not thought to have been long as the director reportedly had been in the room earlier in the day. The director corrected the violation immediately by placing a second staff into the classroom. 1240-04-03-.06 SUPERVISION. Corrective Action: This was corrected on site and observed corrected during a follow-up visit on March 26th. Director states she was short staffed but will not allow this to happen again in that an extra staff will always available to remain in compliance.
