Observation: During an unannounced complaint visit on 6/21/22, Mr. Allen Swank disclosed to Karen Cullum of the Department of Human Services and Licensing Consultant Tracee Garner that in April of 2022 an employee disclosed to him information that she received from a parent noting that an educator had hit a child on his head. Mr. Swank noted neither he nor this educator reported this to the Department of Children Services as mandated per licensing rules. Tenn. Comp. R. & Regs. 1240-04-01-.10 DUTY TO REPORT CHILD ABUSE AND NEGLECT. Corrective Action: The agency was cited for the violation. LC gave TA by reviewing the rules for Duty to Report Abuse and Neglect. A TAM referral will be made for additional training.
