Summary of Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Complaint Investigation. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. A two year old male child had to receive medical care after playtime with an educator that resulted in a dislocated elbow.

See Below for Detailed Information Observation: There is no documentation of a school age child reporting to the Director that inappropriate music with adult content was played in his school age class. 1240-04-01-.09 INCIDENT REPORTING. Corrective Action: Director documented the incident and will share it with the parents/authorized guardians of the school agers present in the class where music with adult content was played within 5 days. Observation: Music with adult content was played in the school age class. 1240-04-01-.15 PROGRAM, LANGUAGE, AND LITERACY DEVELOPMENT. . Corrective Action: Director met with staff and shared the licensing rules to ensure staff understand any music without adult content that is not developmentally appropriate for children cannot be played.

See Below for Detailed Information Observation: The high chair equipment manufactures instructions for use were not followed; the toddler was not strapped in the high chair. 1240-04-01-.14 EQUIPMENT FOR CHILDREN. Plan of Corrective Action: Director checked all classes to ensure all equipment owner manuals were available in each class and reminded staff to follow the owner’s manual for all equipment used for child care. Observation: The pre-enrollment onsite visit is not documented and cannot be determined by staff. 1240-04-01-.05 OWNERSHIP, ORGANIZATION, AND ADMINISTRATION. Plan of Corrective Action: Director notified administrative and enrollment staff to ensure that all children files are documented with pre-enrollment visit dates. All parents must make an onsite pre-enrollment visit. This date must be documented in the child file. Observation: New hire staff files did not contain documentation of physicals, verified references, and education. 1240-04-01-.08 RECORD KEEPING. Plan of Corrective Action: Staff was sent to get the required qualifications. The director must have a system in place to ensure that all staff qualifications are on file prior to allowing staff to start child care duties. Observation: Sign in and out sheets for March 18 and March 19, 2019 have children not signed out of the agency. 1240-04-01-.11 SUPERVISION. Plan of Corrective Action: All errors on logs were noted and discussed with Director and Designee. Designees were assigned by Director to monitor the logs to ensure that each parent sign in and out of the agency with the time, printed name and signature at arrival and departure. Observation: Comprehensive first aid information is not posted in the Toddler room. 1240-04-01-.12 HEALTH AND SAFETY. Plan of Corrective Action: Director provided educator with comprehensive first aid and cpr information. The information was posted in the toddler class areas.
