The management of the agency shall maintain a system that enables all children in the agency’s care to receive a level of supervision of their status and activities that is appropriate to their age and their developmental, physical and mental status so as ensure their health and safety and that allows agency personnel to know the whereabouts of each child in their care. (b) Children six (6) weeks of age through nine (9) years of age: 1. The adult must be able to hear the child at all times, must be able to see the child with a quick glance, and must be able to physically respond immediately. Observation: Agency self reported leaving a child in the gym. PE investigated self reported complaint and determined that teachers did not communicate the correct number of children during a transition to the main building from the gym thus resulting in one child being left behind in the bathroom during a transition from one building to another.

Observation: Agency self reported leaving a child in the gym. Upon Program Evaluator reviewed child class rosters and determined that the child was not on a clearly defined class roster and thus resulting in one child being left behind in the bathroom during a transition from one building to another.

(b) The director, with the guidance of the board or owner of the center, shall be responsible for supervision, training and evaluation of the staff, the program and the day-to-day operation of the center. Observation: Agency self-reported leaving a child in the bathroom. The director is ultimately responsible for supervision of staff and daily evaluation of performance in order to ensure the safety of children.
