Because of the possibility of Sudden Infant Death Syndrome: 1. Infants shall be positioned on their backs when placed in a crib for sleeping. 2. In order to avoid the risk of smothering, soft bedding for infants is prohibited. 3. Infants shall not be wrapped tightly or swaddled in blankets for sleeping. 4. Infants shall be touched by a caregiver every fifteen (15) minutes in order to check breathing and body temperature. 5. Pillows shall be prohibited for infants. 6. If a child appears not to be breathing, the agency must immediately begin CPR and call for emergency medical assistance. 7. Before any caregiver can assume care giving duties of any type in an infant room they shall be oriented in the foregoing SIDS procedures. Observation: On November 8, 2011, the date of your assessment, it was observed that there was no physical touch of sleeping infants at anytime during which the assessment was being completed.

See Below for Detailed Information 1240-04-03.02 DEFINITIONS FOR SUPERVISION (46) Supervision For the purpose of this Chapter, when children are not within the direct sight and sound of an adult, the term "supervision" means the following requirements: (a) Children six (6) weeks of age through nine (9) years of age: 1. Adult must be able to hear the child at all times, must be able to see the childr with a quick glance, and must be able to physcially respond immediately. 2. Exception during mealtime: An adult must be in the direct sight and sound of children ages six (6) weeks through five (5) years, not in kindergarten, while the child is eating. During assessment on December 14, 2009 it was noted by assessor, Lynn Spearman that a child was left alone eating in the 2 year room while the main teacher went into the bathroom, and the second caregiver was attending to other issues in the classroom.

At least one (1) adult shall be awake and supervising the children in each room/sleeping area. During unannounced complaint visit wheras it was alleged that a parent/guardian came in around 01:00 and no adult/caregiver was supervising the children, when ccpe Linda Lloyd arrived at this agency at approx 01:04 I found the same situation as reported in that there was no caregiver in the room where the 3 year olds were sleeping. To be in compliance a caregiver must be in the room supervising the children and also able to physically respond if needed. CCPE found no caregiver in the room. It was noted that the caregiver for this room did "peek around the corner" at this worker, but never asked if she could help me or ask me what ccpe was doing in this room. Agency was cited for lack of supervision on check sheet, 1240-04-03.06 1 b. Supervision procedures. Linda Lloyd Put on notice was mailed on 09/08/2009. On 10/05/2009 put on notice not responded back on. Phone contact made this date to Ms. Grant advising her unless this is received by email today, another notice will be sent for failure to follow through on an cited complaint.
