Summary of complaint visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and to investigate a complaint If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. During a unannounced visit to investigate a complaint after LC Utley completed a walk through of each classroom it was determined that in some of the classrooms tables are not cleaned after meals, floors not being swept after meals, sinks that have dried soap and dirt in the them and children's restrooms that are not clean. During a unannounced visit to investigate a complaint LC Utley completed a walk through of each classroom it was determined that a few of the classrooms had outlet covers that were missing. This had been discussed with director by the LC on previous visits to the agency. During an unannounced visit on 5/15/26 to investigate a ratio complaint after LC Utley reviewed the name to face roll call sheets it was determined that on 5/14/26 the infant class was out of ratio for a period of 16 minutes. The ratio for that class is 1:4. The class had a ratio of 1:5 during that time. During an unannounced visit on 5/15/26 to investigate a ratio complaint after LC Utley reviewed the name to face roll call sheets it was determined that on 5/13/26 the infant class was out of ratio for a period of 56 minutes. The ratio for that class is 1:4. The class had a ratio of 1:7 during that time.

During an unannounced visit on 4/20/26 to investigate a ratio complaint in the infant class. While gathering ratio's LC Utley observed an child eating snack the supervising educator Nicole R. seated in a chair with her back to the child that was eating. While the child was still eating the educator was giving instructions to the other children in the classroom. There was an educator working in a classroom whose background check had expired. The educator did not get reprinted within the 5 year period as required by licensing rules. During an unannounced visit on 4/20/26 to investigate a ratio complaint in the Infant class. While completing ratio checks LC Utley observed educator Nicole R. who was the only educator in Pre K class sweeping the floor while a child was eating snack.

During an unannounced visit on 4/9/26 to investigate Food service and Inappropriate Discipline complaints after LC Utley spoke with the director Tamara M. who had spoken to the Kemarion S. the educator named in the complaint the educator stated he did grab the child's arm but not forcefully. The director also asked the child about what happened. It was determined that inappropriate discipline was used on a 4 year old child.
