Violation

1. Ratio and group size was in violation during the observation. 1:25 was observed; 1:16 is required (while preschoolers are present). (0520-12-01-.06 (4) (h) on page 16) 2. The physical facilities was in violation because the room was measured for 22 children to be present at one time. There were 25 observed to use the space at one time. (0520-12-01-.12 (3)(a) on page 37) 3. An adult that is listed to pick up the child must sign the child out of the program. Older siblings/students at the school are not allowed to be responsible for picking up a child. The child must leave with the correct adult that is listed on the transportation of that child.
