See Below for Detailed Information Observation: The tables in at least two (2) classrooms were not cleaned or sanitized prior to the children eating snack, which is required. 1240-4-3-.11 FOOD. Corrective Action: Agency will retrain staff and ensure that tables are properly cleaned. Observation: A caregiver in the 4-5s room held a child by the wrist and held it too tight leaving a mark on the child's wrist, while moving the child from one area to another in the room. A caregiver should allow the child to come to them or to a specific area. 1240-4-3-.09 PROGRAM,. Corrective Action: Agency received new training from CCR&R on child behavior and management.

See Below for Detailed Information Observation: During the week of a holiday, 12/26/17 - 12/29/17, the preschool and school age classes were combined all day and the ratio for the two (2) groups together was over ten (10) children. 1240-4-3-.06 SUPERVISION. Corrective Action: The agency will not combine the Preschool and School age groups unless they have 10 or less children in the group. Observation: A new caregiver hired on 11/7/17, was missing proof of new employee orientation and proof of high school graduation, which is required. 1240-4-3-.07 STAFF. Corrective Action: All staff files will be complete and all documentation will be available for review. They will use the checklist given to them by DHS to ensure all files are complete for new staff. Agency found the new employee orientation documentation (1/8/18) and they got another copy of the education documentation (1/10/18) and sent it to the PE on 1/11/18.
