Summary of Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. PS observed footage from incident report complaint and witnessed educator leaving the classroom unattended with 3 children left alone.

The strategy for investigation includes but is not limited to: • Staff interviews with all infant educators and director • Review of video footage at time of alleged incident • Review of staff trainings • Reported to DCS Staff did not maintain direct supervision of children during meal time. Safe sleep: During review of video footage there was evidence of blankets and boppies in crib with infants. Provider failed to report incident within required time frame. Staff failed to use restraints with an infant in a high chair. Failure to provide accurate care for infant.
