Garbage observed to be in need of cover
Licensor spoke with staff regarding additional lighting while children rest.
Licensor observed child cot placement to lead to possible sight and sound supervision concerns. Staff and licensor spoke regarding better placement of cot and staff relocated the cot upon discussion.
Licensor observed classroom door automatically unlocking due to sensors. Classroom is currently closed and will not reopen until this sensor is repaired.

On 9/4/25, when entering the PS Prep classroom, the children were observed sitting together having morning snack. The staff in the room had her back to the children. In the Toddler Room, the children were finishing up morning snack. The staff in the room had her back to the children.
White bottles in classrooms do not contain labels. Cleaning supplies located on the floor in the Toddler classroom.
The program doors are part of an access control system, which includes a sensor and push to exit to monitor entry/exit. The main entrance door and the PS 2 exit doors were observed remaining unlocked after the exit sensor was triggered. The sensors were activating inconsistently and opening/closing without motion being detected. On 9/3/25, the Infant Room door, which leads to the main entrance, was observed propped open. The door to the PS 2 classroom leads directly to a main road.
Program does not complete this with families prior to enrollment.
Children in the Toddler classroom were eating during the visit. At least 1 staff was not sitting with them while eating.
Indoor playground floor contained dust/debris inconsistent with time of visit, closed rooms contained cups, expired food, trash, etc. SA classroom has bin of dirty clothing.
Cots were placed directly against program walls and/or directly against program furniture. Furniture contained toys and other items.
Safety surfacing is uneven, overgrown weeds entering the play area, basketball hoop is broken, and bounce house is placed in the middle of play area.
The center uses an app for children's emergency information with each classroom having an iPad. The Infant and Toddler rooms did not have their tablets in the classroom.
In the SA bathrooms, paper towels are not within arm's reach of the children. The soap dispensers and hand sanitizer stations throughout the center were also noted to be not working and/or needed to be restocked.
The PK classroom snacks were observed together in one bin. A child this classroom has a peanut allergy. Although the center is nut free, combining snacks is not making the proper provisions.
Facilities issues were noted throughout the center. Toddler and PS bathrooms sinks were not all working, exposed wires in PS 2 room, SA bathroom toilet seats broken, 1 sink not working, most rugs in classroom are not slip resistant, broken cabinet in SA classroom, broken tricycles, toy structures, and holes in walls of indoor playground.
The center uses an app for attendance with each classroom having an iPad. The Infant and Toddler rooms did not have their tablets in the classroom.
The infant room first aid kit does not contain a cold pack.
During the visit, a child in the PS classroom was engaging in challenging and unsafe behavior. It was reported the child is receiving special education services however the child's file did not contain any information pertaining to the child's IEP, developmental needs, etc.
Garbage receptacles in SA classroom and PS 2 classroom were not lined. Receptacles need to be cleaned.
Classroom arrangement does not allow for clear pathways. In the Toddler bathroom, mops, brooms, etc. were propped against the wall near the entrance. Similar was observed in the SA classrooms and PS 1 classrooms. Classrooms do not allow for separation of quiet/noisy activities.

Reusable sponges were observed being used in the Infant and Toddler room.
Two chests in hallway are used for extra clothing. Storage chest lids do not contain hinges.
SA classrooms have minimal materials and what is present, are not age appropriate. Materials in other classrooms are also minimal and/or not accessible.
Food allergy information was not posted in the Infant room. The information posted in other room did not have the children's classrooms accurate.
Graphic evacuation plans in the rooms are not accurate and/or did not contain the plan for the room in which it was posted (i.e. PS 2 room posting was for SA classrooms).
A review of injury reports noted inconsistent documentation and notification in adherence to the regulations.
