Evacuation cribs observed to be obstructed by classroom items resulting in them not being readily available.
Licensor observed staff member with back turned to classroom while additional staff was in the bathroom assisting students. Due to this children were out of sight supervision.
Licensor observed outlets within child reach in need of covers.
Infant first aid kit in need of plastic bag.
Spray bottle observed on the floor next to changing table.

Licensor observed classroom materials to be torn and in need of replacement. Licensor observed outdoor materials to be stacked on a wall. This caused a tipping hazard on the playground area. Staff spoke with staff about relocating materials to the shed.
Licensor observed paneling on cabinet to be peeling and chipping
Bedding in cubbies observed to be touching and not stored in bags or comparable means.
Bottles in all infant rooms in need of labeling.
Child with medications on site in need of completed medication authorization form and doctor documentation.
Licensor observed toddler room to have two broken toilets in restroom
licensor observed highchair with missing shoulder straps. Licensor spoke with staff and assistant administrator advising the chairs that required straps must be used for safety.
Licensor observed Preschool room to have one broken sink and a toilet out of order.
Medication for students with care plans and documentation missing on site.
Licensor observed highchairs blocking emergency exit obstructing exit for evacuation cribs.
Licensor observed bottle with milk in childs cubby. This bottle had no label or documentation on the usage of the bottle.
While choke prevention gauge was on site in the administrators office. Upon request in the infant and toddler rooms staff were unable to locate this item and were unsure of where one would be located.
Medication authorization forms were incomplete. Licensor advised assistant administrator the forms must be completed with medication, dosage, frequency and all additional information on the form should be completed and signed by families.
Licensor reviewed attendance in all classrooms. Attendance for infants and toddlers were not accurate upon review
Bedding on cribs observed to be loose fitting.
Licensor observed a staff interaction with child who was in need of redirection. Staff observed to use a assertive tone of voice and child became upset. Licensor observed infant and toddler visibly upset in respective classrooms (crying, seeking adult to hold them) Staff were not observed to console the students while licensor was in the room
Classrooms missing emergency information for students.
Licensor observed clothes used for cleaning to be in classrooms on garbage receptacle and sinks.
Toddler programs in need of evacuation cribs. Licensor spoke with staff and assistant admin on site regarding regulation for evacuation crib being in classroom with children under the age of two.
