Spill kit needed in vehicle. Licensor reviewed first aid kit and observed over the counter medication to be stored inside.
Licensor observed vehicle to be in need of upkeep. Licensor observed trash, open food, broken crayons and wrappers throughout the vehicle
Staff were unable to access medication and documentation for children upon request.

Upon arrival licensor observed one staff person with 18 students. Staff person reported the second staff on site was moving the vehicle around back to transport students to school. . Due to one student being 4 years old on site the ratio of the group is decreased to 1:10.

Licensor observed seats in van torn and duct taped. Additionally back seat in van buckle not accessible. Out of order sign was requested for this seat.

Throughout visit staff and children were not actively engaged. Students were observed to be on electronic devices (phones and ipads) for the duration of the time licensor was present. Licensor spoke with owner regarding engaging students in activities and curriculum

Licensor reviewed care plan requirements for students with emergency medications such as epi pen and inhalers.
Licensor observed two seats where buckles were not in working condition. Licensor requested signage be posted to ensure students do not use the seats. Licensor completed walk through of three vehicles. Two vehicles were maintained and approved to use for transportation. Additional vehicle had torn seats, broken seat belts, arm rests, exposed metal, and parts duct taped in place. Licensor advised this vehicle was not safe for transportation.

Licensor Jessica Ullrich observed three vans. All three van have seat belt issues.
While observing the children playing on the large gym mat, staff members were seen sitting of to the side of the mat disengaged from the children and the activity they were playing. One staff was observed sitting with a group of girls and positively interacting with them.
Staff showed licensors attendance/emergency contact information that was in the office and stated that she had not filled it out for today.
Licensor observed furniture/materials throughout the program to need some repair.

Licensor observed children to be sent to the bathroom on their own without sight and sound. Staff did not ensure children followed handwashing procedures.

Licensor Jessica Ullrich observed transportation vehicles to need repair (please see her report for findings). Licensor observed program to be in need of maintenance as the carpeting was torn and ripped, boy’s bathroom sink was not fully working (no hot water), bathrooms were not clean.
