Licensor observed toddler classroom to have one evacuation crib stored in the closet. The crib was not readily accessible due to the positioning of the crib in the closet. Staff moved the crib for it to wheel in and out easily. An additional evaucation crib is needed as the room is all children under the age of 2 years old.
Licensor observed infant hand washing sink to be containing all used bottles from throughout the day. Licensor advised the sink cannot be used for cleaning bottles as it is the designated handwashing sink for diaper changes.
Licensor observed Toddler 2 to need choke saving poster.
Licensor reviewed first aid kits in all classes. First aid kits observed to be in need of replenishing as well as licensor removed tylenol from one first aid kit. Additionally, all classrooms are in need of thermometer.

Licensor observed students napping out of sight supervision due to placement of cots and staff. Licensor spoke with staff in the classrooms to discuss best way for staff to position during rest time to ensure all students are supervised by sight and sound.
Licensor observed a toilet broken in preschool, and the sink counter top cracked and broken in the toddler room
Current license could not be located by staff. Licensor advised a copy of the license must be posted in conspicuous location of the program.

Licensor observed taller shelving unit and classroom materials arranged in a way to inhibit sight supervision in the preschool classroom
Licensor observed two crib sheets to be loose fitting and requested the sheets be tightened or replaced.
Licensor observed two inhalers on site in need of original packaging or medical orders.
Licensor observed students with medications for both asthma, allergies, and febrile seizures in need of care plans.
Medication on site observed to be expired as well as not labeled or in original packaging.
Infant classroom and toddler 1 evacuation cribs were not empty. They were emptied while licensor was on site.
Unable to view safety drill log at visit- admin was offsite and staff in office could not provide licensor with documentation.
