Licensor observed the following items to be in need of correction: Power strip was plugged into outlet underneath students chairs for them to charge devices. Children were engaged in a water activity close to this outlet causing potential hazard. Licensor observed materials to be stored in hallways and on top of shelving/ cubbies which created hazard from clutter. Bathroom tiles were broken and in need of repair

While completing visit licensor observed students to be out of sight and sound supervision in school age room. Licensor spoke with adminsitrator who advised staff they must have one staff in each of the spaces to ensure sight supervision.
