3270.113(a)(1) · Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.
During naptime, in the Younger Preschool Room, Staff Person #1 identified 5 children in their primary care group and Staff Person #2 identified 3 children in their primary care group. There were 2 classes combined for naptime with a total of 14 children. Neither staff person included the other staff person's (on break) 6 children as part of their primary care groups.

Children in the Prekdg class were not regularly taken outdoors daily. Staff indicated that they take children out in the afternoons, but not always in the mornings. While children go to the gym some mornings for gross motor play, there are many children who are picked up before children go outside to play on the playground (4:15 or 4:30), so those children do not go outdoors daily, weather permitting. Class schedules include a morning and an afternoon outdoor play time.

The parent provided the weighted blanket and gave written permission to use it, however, no written instructions/recommendations from a medical professional regarding the recommended weight and usage were provided, nor was there any information on the use of the weighted blanket in an IEP at the facility.

Mulch is present around the preschool equipment, which is accessible to the toddlers as there is no fencing dividing the play space.
