On the morning of 2/18/25, a parent dropped off her 2 young school aged children. Staff Persons #1 and #2 were supervising the group of 26 school aged children (mostly young school agers). Staff Person #1 at that time, had 14 children in her care group and Staff Person #2 had 12 children in her care group. The parent stayed there with her children until Staff Person # 3 arrived at 7:45am. The school age room was out of ratio by 2 children, from 7:34 - 7:45am.

On 7/17/24, Staff Person #1 aggressively tossed Child #1 down on his rest mat so that he would stay on his mat. Child #1 reportedly does not fall asleep for nap. Non-nappers are taken to an "awake" room at 2:00pm, however Child #1 and other non-sleepers are expected to stay on their mats, with nothing to do, from 12:45 - 2:00.
