Provider self reported on 12/10/2024 Child #1 (DOB see LIS code sheet) was left unsupervised on the playground for approximately 5 minutes. Staff Person #1 (DOH see LIS code sheet) and Staff Person #2 (DOH see LIS code sheet) stated that Staff Person #1 was standing at the end of the hallway closest to the preschool room while Staff Person #2 was standing at the other end of the hallway by the playground calling children's names and having them line up in the hallway as Staff Person #1 checked off the children's names on the CSR form. Staff Person #2 stated that once all the children were inside Staff Person #2 did a visual sweep of the playground and closed the door while Staff Person #1 counted 20 children and they all proceeded to walk into the classroom. Staff Person #1 and Staff Person #2 stated that they never noticed Child #1 leave the group and go back out to the playground. Staff Person #3 observed Child #1 standing outside the window of the DPS A classroom. When Staff Person #3 realized Child #1 was alone on the playground they notified Staff Person #4 and #5. Staff Person #4 and #5 immediately retrieved Child #1 from outside on the playground and returned Child #1 to the preschool room. Staff Person #1 and #2 did not know the whereabouts of Child #1 for approximately 5 minutes.

It was observed on 12/20/2024 in the Toddler B classroom liquid or powdered soap was not available to be used for handwashing. Staff in the room stated that the soap dispenser was broken a couple of weeks ago and they have just been using baby wipes and running water to wash their hands and those of the children before eating and after diapering. Staff from the facility self-reported that on 10/30/2024 during nap time in the DPS A classroom Staff Person #1 used their body to pin Child #1 (DOB see LIS code sheet) to their cot because Child #1 was up roaming around the room instead of resting on their cot. Staff Person #2 described Staff Person #1 as using their body weight to hold Child #1 down on their cot and when the child would move Staff Person #1 would pick them up by an arm and a leg and "flop" the child back down onto the cot. Staff Person #2 also described Staff Person #1 using their hands to hold Child #1's head still when the child would shake their head back and forth in an effort to self-soothe. Staff Person #2 also stated Staff Person #1 wrapped Child #1 up in their blanket in an effort to restrict the child's movements. Staff Person #2 stated this lasted for 30-60 minutes.

By doing so, the facility was without running water in most of the facility (except for one portable sink in the Toddler A classroom) from approximately 10:30 am until 12:30pm when the plumbing was fixed. During a complaint investigation conducted on 9/12/2024 it was verified that a chair being stored in the bathroom in the DPS B classroom posed a tripping hazard for an incident that occurred on 8/27/2024. On an incident/accident report that was provided to Child #1's parent it stated Child #1 " was in the bathroom and tripped over the chair and hit his face on toilet" which caused severe bruising to the child's face.
