Staff #1 was observed at the door of the PK classroom with their back away from the children, and at times a few feet from the doorway. Staff #1 was unable to properly supervise children in the bathroom and hall area. During this time, children were also observed in the bathroom changing their own wet pull up and leaving it on the floor. At one moment, while Staff #1 was not directly supervising the children, a child was observed leaving the group and walking partially down the hallway (certification representative directed the child back to the group).
