The trash can in the bathroom did not have a lid that contained paper waste. Facility person #1 and facility person #2 was observed to be caring for children unsupervised. Facility person #1 and facility person #2 has not completed the following pre-service training required prior to caring for children unsupervised: Health and Safety training. Facility person # 1 and facility person #2 did not have emergency plan training at the time of initial hire.
