During the investigation, it was verified that on 12/30/24 Child #1 tipped a bouncer chair and fell hitting forehead on the floor causing a bump on the forehead. Staff #1 did not complete the injury report until 12/31/24 and it was given to the parent on 1/2/25.

During the inspection, it was verified that Child #1 was engaging in inappropriate conversations with other children in the Little Tykes Playhouse. Staff #1 and Staff #2 did not directly hear or see what was happening inside the playhouse but were told by several children. A subsequent investigation, by an outside agency, determined through interviews that inappropriate touching also occurred.
