At that time, both doors to the kitchen, which is located off of the Main Room, were unlocked and propped open, and several cleaning agents labeled "keep out of reach of children" were observed in the lower cabinets of the kitchen, which were accessible to children. In addition, an unlabeled bottle of cleaner was observed in the kitchen.

The stove was not equipped with safety covers on the knobs, creating a hazardous condition where children could potentially operate or come into contact with cooking controls or heated surfaces.

An allocated unannounced monitoring was conducted on 12/29/2025. At that time, required first-aid kits were not present or were incomplete in several measured child care spaces. No first-aid kit was present in the Nursery, Room 23/25, or Room 10/12, where children were receiving care. When a first-aid kit was brought into Room 10/12, it was incomplete, missing tweezers and soap. A first-aid kit located in Room 17/19 was also incomplete, missing tweezers and scissors.
