    During a complaint investigation started on 10-29-25, the operator self-reported that a child tripped over an exposed tree root on the outdoor play space and bumped their head on 10-29-25. During a visit at the facility on 11-4-25, multiple tree roots were observed to be exposed, creating a tripping hazard for the children.
