3270.113(a)(1) · Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.
During staff interviews conducted as part of a complaint investigation, it was reported Child #1 left the playground and entered the facility alone and unsupervised on 3/18/22 between 5:30 and 6:00 PM for a period of time between 3 and 15 minutes. On 3/29/22 at 1:15 PM, Staff #1 was unable to name 2 of the 3 infants assigned to her supervisory group. Staff #1 stated it was her first time in the room and she was not familiar with the children..

During naptime in the PS Room on 3/29/22 at 1:35 PM, cots being used by sleeping children were observed to lack 2 feet on at least 3 sides.
