Staff person #3 was unable to properly supervise the child in the restroom. To access this bathroom, one must walk out of the gym through a door and into an office area, walk approximately seven feet to enter the restroom. Staff person #3 was observed located on the opposite side of the gym room with 4 preschool age children.
