On 9/22/22, Staff 1 reported that at approximately 12:00pm Child #1 was having a "tantrum." Staff #1 reported witnessing Staff #2 picking up the rest mat belonging to Child #1 and "hitting Child #1 in the head with it" while Child #1 was sitting upright. Staff #1 reported that approximately 5-10 minutes later, Child #1 was still upset. Staff #1 reported that Staff #2 got a spray bottle out of the cabinet. Staff #1 witnessed Staff #2 walk past Child #1 and spray Child #1 2-3 times in the face. Staff #1 reported Child #1 became more upset after being sprayed by Staff #2. When interviewed by Staff #3, Staff #2 acknowledged "spritzing" Child #1 with the bottle. Staff #2 stated the spray bottle contained only water.
