An unannounced inspection was conducted on 5/6/26. During the inspection Cert Rep observed several occasions where the PS children were not being supervised by staff person #1. During the morning bathroom break all the PS children came back into the classroom with staff person #1 and child #2 was locked in the bathroom stall unattended. Staff person #1 went back into the bathroom to get child #2 and left the preschool children unattended. During lunch, all the PS children were eating lunch in the lunch area and Cert Rep came out of another room and observed child #2 underneath a classroom table alone. Cert Rep walked child #2 back over to the lunch tables. Cert Rep observed child #1 run away from the lunch tables during lunch and ran into the main classroom into a playhouse. He could not be seen by staff person #1 or any other staff while he was in the playhouse.

Staff person #1 was screaming at the children and directing the children loudly in a harsh tone. Staff person #1 repeatedly told Cert Rep in front of the PS children, "that they wanted to quit, didn't know why they were here anymore and is frustrated because they never have help." Staff person #1 yelled repeatedly for child #1, "To go to their calming house and sit."
