At the time of the inspection on 12/9/25, the thermostat located at the back of the facility by the bathrooms read a temperature of 58° F. The thermostat located in the Pre-K room read a temperature of 64° F. Staff person #2 acknowledged that the previous night, the thermostat was set for 68° F. When staff person #2 arrived at 7:30am on 12/9/25, the thermostat was reading 58° F. Staff person #2, as well as additional staff in the facility, acknowledged that the heat has not been working for a few days. The certification representative measured the temperature in the classrooms, and they measured 64 ° F. Facility person #3 denied that the heat was not previously working, as reported by staff. Facility person #3 stated "that the heat was working fine on Friday. The heat was working on Monday as well. It wasn't until Tuesday morning we they noticed a drop in the temperature despite raising the temperature on the thermostat to 72° and we contacted the landlord about it at 8:51am."

The updated policies must also reflect the need for parental consent for any child to leave the classroom to receive outside services. It will be sent to DHS for review prior to training any staff. Both the updated supervision policies and new supervision procedures must be submitted to the Northeast Regional Office for approval prior to implementation. Once approved by the Department, the director will ensure that all current employees and any new hires are trained on the policies and procedures, and it must be documented in their files. This date of correction will be when the policies and procedures have been approved and all staff will be trained. 3.) The legal entity must arrange for all facility staff hired as of the date of the training to receive a minimum of 3 hours of PQAS-approved training regarding supervision and ratios. The legal entity must receive DHS approval of the training content prior to scheduling the training. This date of correction will be when all staff complete the supervision and ratio training. 4.) For a period of 3 months, the facility shall have no occurrences of non-compliance related to supervision of children. The operator shall demonstrate compliance on unannounced visits over the next 3 months. This date of correction will be three months from when an acceptable plan of correction is received.
During the inspection on 9/15/25, at 8:19am, staff persons #3, #4, #5 and #6 had 12 mixed-age children in the preschool room. When the certification asked each staff person which children they were responsible for, all staff acknowledged that they do not take accountability for specific children until they go to their classrooms at 9am and then will identify their primary care groups, but it is not done prior as children are being dropped off in the morning in the preschool room. At 9am, there were 26 mixed-age children present in the preschool room and not one staff person was able to identify which specific children they were responsible for. At 9:38am, the certification representative observed members of an outside agency removing child #1 from the Preschool classroom. When the certification representative questioned the Director, who was present in the Preschool room, where the child was going, she was unaware of the outside agency removing the child. The Director questioned the certification representative saying, "they left with the child?" The director then walked to the hallway where child #1 and the outside agency was. The outside agency claimed the child was overstimulated. The Director acknowledged that they do not have written consent from the parent for the child to be removed from the classroom to receive outside services.

This number and age of children requires there to be 6 staff present at all times.

During another inspection on 7/16/25, Staff person #6 was observed unsupervised in the young toddler room while changing the diaper of child #3. The daily report from 7/3 showed that child #1 was checked into the facility at 7:08am and the diaper was not checked/changed until 10:01am. The diaper was not checked/changed again until 12:34pm. The child went down for rest during 12:59-3:06, woke up and had snack and was not changed again until 3:42pm.

At the time of certification representative's arrival on 6/20/25, staff person #1 opened the facility door, which is located in the foyer of the facility, and was observed to be holding child #1. Upon arriving back in the infant room, a second infant was observed in a bounce chair. This infant was left unsupervised during the timeframe of staff person #1 leaving the room to open the door to returning, approximately 1 and 1/2 minutes. During another inspection on 7/16/25, staff #7 was observed with and identified the 8 children in the preschool classroom (7 preschoolers and 1 older toddler). Staff #8 then arrived with 6 preschool-aged children. This staff was asked to identify the children assigned to this staff, but only identified 4 of the children as assigned in the group. Two children were not assigned to the responsibility of any staff person present in the classroom.

Staff person #7's voice was heard numerous times while the certification representative was in other classrooms with the director. The director acknowledged that staff person #7 needed to lower the level of her tone towards children.
