At the time of the inspection, in the outdoor play space, an easel with exposed screws was observed. This was in poor repair. Two green plastic turtle sandboxes were observed. The lid of one sandbox was cracked, and the other sandbox had a crack in the base causing a hole and creating pinch points.

CONTINUED NON-COMPLIANCE: During the inspection on 4/21/25, a camera wire was observed hanging and was not properly secured to a surface. The outdoor play area had plastic cups, ramen noodle wrappers and other trash laying on the ground. An acceptable plan of correction was received stating the correction would be implemented by 4/24. An inspection occurred on 5/30/25, the outdoor play area had snack pack containers and other trash laying on the ground.

3270.106(a)/3270.119 · Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child. Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.
At the time of the inspection, child #7, an infant, was observed sleeping on his stomach in a crib that was designated for another child. Staff person #2 admitted to placing child #7 on his stomach to sleep. This sleeping position is not recommended by the American Academy of Pediatrics.

The three climbers and slide in the play yard shall be made inaccessible and will not be used by the children until such time as the loose-fill impact-absorbing material meets the recommendations of the United States Consumer Product Safety Commission. For this piece of equipment, there needs to be at least 6 inches * of loose-fill impact-absorbing rubber mulch under the equipment. This portion of the plan shall have an immediate correction date. 2.) Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The equipment must be anchored firmly and be in good repair. The operator shall obtain more loose-fill impact absorbing material to bring the depth into compliance with the recommendations of the United States Consumer Product Safety Commission. This portion of the plan shall include a date for when the mulch will be added to bring the depth into compliance.
During the inspection, in the play yard, the smaller embedded climber, the slide, the larger embedded climber and the circular climber had fall heights of 30", 34", 34" and 46" respectively. These pieces of equipment only had 2 inches of loose fill rubber mulch under them and surrounding the structures.
