On 6/4/21 Staff person #1 posted a partial picture of Child #1's health assessment which included the child's health history and medication on the parent's personal face book page. The parent did provide proof of the picture posted. The child was easily identifiable because the provider identified the paperwork posted as the parent's son's form.
