All required clearances for staff must be updated at least every 60 months.

Staff #1 had current clearances on file on 5/12/2026. However, the updated National Sex Offender Registry (NSOR) clearance on file for Staff #1 was dated 4/30/2026. The previous NSOR clearance on file for Staff #1 was dated 7/27/2020. Staff #1 verified that they had direct contact and routine interaction with children while this clearance was expired.

During the Renewal Inspection on 5/12/2026, a large plastic playset in the outdoor playground was observed to have several cracks on the inside of the playset. This left sharp edges exposed.

However, the log showed two lapses during the previous 11 months in the requirement to manually test the fire detection system at least once every 30 days as follows: The alarm was tested on 6/19/2025 then not again until 8/18/2025 and the alarm was tested on 1/21/2026 then not again until 2/23/2026.
