An unannounced inspection was conducted on 11/14/2024. Upon arrival, at approximately 10:55 AM, Staff #1 was the only facility person present. There were 5 children present at this time. Staff #2 arrived at the facility about 25 minutes later. Staff #1 reported that there was a 2nd staff person present from opening in the AM until 10:00 AM.

On 11/4/2024, the following was observed on the outdoor playground: Rotted wood on two of the outside steps leading from the back of the building to the playground and lattices that they were cracked in several areas, which left sharp edges exposed.
