On 6/8/2026 at approximately 12:10pm, supervision and ratio was being assessed at the facility. At the time, Staff Person #1 identified two young toddlers as being under their care and supervision. When Staff Person #2 was asked for whom they were responsible, they named the same two young toddlers, leaving the remaining five preschool-aged children without supervision. Staff Person #1 then told Staff Person #2 that they were to be responsible for the preschool children while the two young toddlers were at the top of the play area near Staff Person #1. Staff Person #2 did not seem aware of this but was then able to identify the remaining five children.
