    3270.113(a) · Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Children must be supervised at all times.

    On Monday July 25th, 2022 at approximately 12:40, Child #1 was left unsupervised when he walked out of the room/building unknown to Staff #1 and Staff #2. He was in Staff#1's Primary Care Group. Child was found outside by a Construction worker who returned child to facility. It is estimated child was unsupervised for approximately 2 minutes.
