LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 5 and #8 may not work in a child care position at the facility. PROVISIONAL HIRE UNSUPERVISED: A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #5 and #8 may not work in a child care position at the facility.
Staff #2 had an NSOR dated 9/8/25 and an FBI clearance dated 9/8/25, which were not obtained before working with children. Staff #3 had a child abuse clearance dated 7/10/20 and updated 1/20/26, an NSOR dated 2/27/20 and updated 4/21/26, and Mandated Reporter training completed 7/10/20 and updated 9/21/25, all of which were greater than 60 months apart. Staff #4 had a child abuse clearance dated 7/8/20and updated 7/11/25, which is greater than 60 months apart. Staff #5 resided out of state within the past 5 years but did not have out of state clearances on file. Staff #5 may not work in a child care position until the clearance is obtained and on file. Staff #8 did not have an FBI clearance on file. Staff #8 may not work in a child care position until the clearance is obtained and on file.
