    During an unannounced complaint investigation on 4-25-24 at 16:45, it was determined that a parent was denied access to the facility when staff did not hear him knocking and the doorbell was disconnected. He called the facility and the number had been disconnected. This writer attempted to call both numbers listed for the facility, and both had been disconnected,

    During a Negative Sanction Unannounced Monitor on 4-15-24, my Supervisor and myself reviewed the file of Staff #2, hire date 3-18-24, and discovered that she had a Volunteer Child Abuse Clearance dated 5-22-23 and she did have a completed Pennsylvania State Police Clearance dated 5-16-23. A Provisional Hire may work in the vicinity of a permanent employee, if they have a Non-Volunteer Child Abuse clearance and a Completed FBI or Pennsylvania State Police Clearance while awaiting the other clearances for 45 days.

Staff #3 is outside the 45 day Provisional Hire time frame. This citation was issued previously in January.

    During an Unannounced Monitoring visit on 1-10-23 it was observed that Staff#1 was out of ratio with a mixed age group of children. In her group were 2 one-year olds, 4 two-year-olds, and 1 three-year-old. For 7 children to 1 staff, and her ratio should have been 1 staff to 5 children.
