Facility Person #1 and #3 may not work in a child care position at the facility until the results of the required clearances are on file at the facility.

A staff person shall administer a prescription medication only if written instructions are provided from the individual who prescribed the medicine. Instructions for administration contained on a prescription label are acceptable.

It was observed on 6/16/2025 an inhaler for child #1 is on-site at the facility. The box in which it was received with the prescription label was not present at the facility. Written instructions from the individual who prescribed the medicine have not been provided.

It was observed on 2/28/2025 the comprehensive general liability insurance certificate secured by the LE does not include the name of the facility or the location information anywhere on the certificate.
